ABSTRACT

Physicians have to play the role of a team leader and counselor and take written informed consent for high?risk surgeries in many cases. This is the first step toward initiating a mutually trustworthy relationship with the patient and family. The situation is more sensitive when vulnerable patients like small premature babies or the elderly are under consideration. In the event of a death, leadership and processes in place become extremely critical. We share our experience and practice pattern during this process, especially suited to India, but the broad principles would apply to most human situations. Hopefully, some of these can be incorporated into the existing training curriculum for team building and the art of effective physician?patient communication that should be intricately woven into the curriculum for the Science of Ophthalmic care.

METHODS: By analyzing and summarizing the experience and enlightenment of the US FDA's drug inspector team, a series of research are launched considering current situation of drug inspectors, and problems in the China's drug inspector team building are analyzed in depth. RESULTS: It is to provide approaches and methods for the construction of China's drug inspection team. CONCLUSION: The construction of China's drug inspection team should combine both long-term and short-term considerations and solve problems step by step. In the long run, professionalism is the orientation. In the near future, before realization of professionalism, the construction of drug inspectors should focus on specialization and develop full-time and part-time inspectors. At the same time, it is necessary to highlight the professional and technical characteristics of drug inspection, reflect the "high-precision" technical level of drugs, and build a team of professional inspectors at the national and provincial levels. In the consideration of the source of the inspectors, it is recommended that the inspectors to be hired from the third party outside the supervision system, and let local pharmaceutical vocational colleges play great roles to develop potential teams of inspectors.

Methods The stratified random sampling method was used to collect data on the flow of village doctors from 2010 to 2015. Thirty-six townships were sampled, which were located in 12 counties from 6 provinces in the eastern, central and western China respectively. Results The average age of sampled village doctors was 31. 25 ± 8. 75 years, and most of them graduated from technical secondary school(57. 21% ). The outflows were greater than the inflows, and the net inflow in the eastern region was highly negative. Among the outflows, those under 60 years old accounted for 44. 78%, and most of them went to medical institutions of higher level. 22. 26% of the outflows were under 40 years old, while 85. 42% of them had college degree or above, and 71. 72% were practicing(assistant) physicians. Conclusions The main force of village doctors is draining, with replenishment difficult to attract. Income and career prospects are the main causes for the loss. A model of rural doctors is thus recommended, combining such major elements as capabilities, motivations and opportunities for reinforcement of the teambuilding.
